Life Disruption or Life Reconstruction: A Study on the Decision-Making Process of Marital and Childbearing Choices Among Gynecologic Cancer Survivors

摘要: 文章旨在从人生进程的视角出发,深入探讨婚育决策过程在妇科癌症幸存者人生进程重塑中的作用。通过质性研究方法,研究团队在2022年11月至2023年6月期间,开展了线上半结构式深度访谈。研究揭示,妇科癌症作为一项重大生命事件,对其幸存者的婚育决策造成了双重压力:健康挑战与社会角色的期待。在此背景下,妇科癌症幸存者作为主动应对者,积极面对这些压力,不仅体现了其在婚育决策上的自主性,也是其重塑人生进程的关键。特别是那些未婚或未育的女性,在生命的关键时刻面临重大决策,这进一步加剧了其疾病与婚育选择之间的相互影响。婚恋自主性作为个体在恋爱和婚姻选择中的自由与自决权,成为人生进程恢复与重建的核心要素。值得注意的是,婚恋自主性并非仅从传统婚育观念向现代婚育观念的转变,更重要的是,它促使幸存者对自身的婚育观念和行为进行深刻反思,并据此做出调整,从而实现人生进程的重构。因此,健康社会工作应为幸存者提供全面而长期的支持性服务,以缓解其疾病与婚育选择之间的紧张关系,帮助其在复杂的医疗和婚恋决策中找到明确的方向。

Abstract: With the increasing incidence of gynecologic cancer, a growing number of survivors are confronting the challenges of post-treatment life as cancer survivors. This study delves into the shifts in the life trajectories of gynecologic cancer survivors, especially in terms of marital and reproductive decisions. Employing qualitative research methods, the study spanned from November 2022 to June 2023, recruiting respondents through online purposive and snowball sampling. Data were gathered via online semi-structured in-depth interviews. Findings reveal that gynecologic cancer, as a major life event, places dual pressures on survivors’ marriage and parenting decisions: health challenges and social role expectations. Notably, unmarried or childless young survivors faced crucial decisions during critical life stages, intensifying the nexus between illness and choices regarding marriage and parenting, profoundly altering their life trajectories. Nevertheless, gynecological cancer survivors exhibited remarkable autonomy in navigating marital and reproductive decisions. They redefine these choices, prioritizing individual physical and psychological needs over societal conventions. In this context, gynecologic cancer survivors’ active coping not only reflects their autonomy in marriage and parenthood decision-making, but is also key to reshaping their biography. The study emphasizes the necessity for comprehensive, long-term support services in health social work. These services are crucial in alleviating the tension between illness and marital choices, aiding survivors in negotiating the complexities of medical and marital decision-making.
